TERMS & CONDITIONS / CLIENT

By booking a consultation, purchasing a service, or submitting payment to Hair As Prescribed Rx LLC, you agree to the following terms and conditions.
1. CRANIAL PROSTHESIS POLICY
All cranial prosthesis units are custom or made
to-order medical hair systems.
Due to the nature of these services:
• All sales are final
• No refunds will be issued once a unit has been
ordered, customized, or constructed
• Adjustments are only provided if included in your selected package
2. CONSULTATION POLICY
Consultations are required prior to purchasing a cranial prosthesis.
Consultation fees are:
• Non-refundable
• May be applied toward your service if stated at the time of booking Clients are expected to arrive on time. Late arrivals may result in a shortened consultation or rescheduling.
3. DEPOSITS & PAYMENTS
A deposit is required to begin any cranial prosthesis service.
• Deposits are non-refundable
• Remaining balance must be paid prior to delivery or installation
• No product will be released until full payment is received Accepted payment methods: __________________
4. CANCELLATION & RESCHEDULING POLICY
We require at least 24–48 hours' notice for cancellations or rescheduling.
• Late cancellations may result in forfeiture of deposit
• No-shows may require a new consultation fee to rebook
5. CUSTOM WORK AGREEMENT
By proceeding with services, you acknowledge:
• You have discussed and approved your desired style, color, and length
• Custom cranial prostheses may have slight variations due to the handcrafted nature
• Processing and delivery times will be discussed during consultation
6. INSURANCE DISCLAIMER
Hair As Prescribed Rx LLC provides documentation to assist with possible insurance reimbursement.
However:
• We do not guarantee insurance coverage or reimbursement • Clients are responsible for verifying their own insurance benefits
• Full payment is required regardless of insurance outcome
7. MAINTENANCE & CLIENT RESPONSIBILITY
Clients are responsible for proper care and maintenance of their cranial prosthesis.
• Longevity depends on proper maintenance and usage
• Damage due to improper care is not the responsibility of Hair As Prescribed Rx LLC
• Maintenance services are recommended for optimal results
8. MEDICAL & SCALP CONDITIONS Clients must disclose any known scalp conditions, sensitivities, or medical concerns.
Hair As Prescribed Rx LLC is not liable for:
• Allergic reactions
• Skin sensitivities
• Medical complications related to underlying conditions
Clients are encouraged to consult with their physician if needed.
9. NO GUARANTEE OF RESULTS
While we strive to provide high-quality cranial prostheses, results may vary based on:
• individual scalp conditions
• lifestyle
• maintenance practices
10. PHOTOGRAPHY & MARKETING CONSENT (OPTIONAL)

11. LIMITATION OF LIABILITY
Hair As Prescribed Rx LLC shall not be held liable for any indirect, incidental, or consequential damages arising from services provided.
12. AGREEMENT
By booking services and/or submitting payment, you acknowledge that you have read, understood, and agreed to these Terms & Conditions.
